Hey Mark! I went with Aetna GoodHealth, which I think is a good value: About $1000 per year for me. I booked through Pacific Prime www.pacificprime.com who were always very thorough in answering my questions; I'd recommend at least trying them for a quotation.
Aetna GoodHealth has a direct billing network in China / Asia. Coverage is worldwide with in and out-patient treatment. In the US it only covers emergency unscheduled treatments. It does not cover regular check-ups. But that works for me, as I just want something for emergencies anyway.
I haven't had to use it (knock on wood), so I cannot speak to how the service is when in need.

We're using AXA-MinMetals, has worldwide coverage with direct billing in China and HK. I have their contact details, buzz me if you want them :)
All the major insurance companies will be at next month's Expat Show. It's once again at the Exhibition Center on Yan'an from Sept. 17-19 and is free if you register online or pick up one of the zillion flyers around town. Www.expatshowshanghai.cn
